I've never seen people go down while descending but I'm sure it happens. I have turned people around during their ascent...more than handful, less than a dozen.
Much of this has to do with willful ignorance...many do not want to throw money at either a guide book or a map prior to heading to Mt. Whitney, and being tired. I know I was exhausted going by there my first trip to Mt. Whitney.
